All of the forms in this section of my Web Site are needed for the loan application process. Please print out these forms and then complete and sign the loan application form and sign all the disclosures.

Please return these to me together with:

  • Proof of income - 2 recent paycheck stubs, 2 years W2s if you are an employee – if you are self employed I will need 2 years Federal Income Tax Returns (with all schedules) plus a Year- to- Date Profit and Loss Statement.
  • Proof of assets in the form of 2 recent months statements from savings, 401K, retirement account or brokerage accounts.
